  <p>An exciting opportunity has risen for a Health and Wellbeing Coach. This position will be to support the Swap to Stop Campaign on our Inpatient Units as well as our staff within the organisation. You will be required to travel to different trust locations but this will mostly be focused around the inpatient and forensic units. You will support people to develop their knowledge, skills and confidence in managing their health, to improve health outcomes and quality of life to support them making positive changes via coaching and through providing access to interventions such as self-management education, peer support and more. There will be a particular focus on going smoke free through utilising nicotine replacement therapy or vapes as quitting tools to stop smoking.</p>
<p>The successful candidate will be compassionate, reflective and self-aware and enjoy working with a wide range of people. They will have good communication skills and a firm belief that people have untapped resources within them, that can be developed by providing high quality, non-judgemental support. They will be proactive with a flexible attitude, keen to work as part of a team and committed to developing themselves in this skilled role.</p>



  <h3>Main duties of the job</h3>
  <ul>  <li>Provide 1-1 support for patients within the inpatient units and forensic units to no become dependant on tobacco and to encourage a healthy lifestyle.</li>  <li>Manage and prioritise a caseload, in accordance with the needs and support required by individuals. Along with a strong awareness and understanding of when it's appropriate or necessary to refer people back to other health professionals/agencies, because the person's needs are beyond the scope of the role</li>  <li>Conduct health and wellbeing tests including a finger prick test for cholesterol, blood pressure checks, height and weight measurements.</li>  <li>Health and wellbeing discussions including smoking cessation, physical activity and alcohol intake</li>  <li>Provide smoking cessation support to employees of Humber Teaching NHS FOundaiton Trust</li>  <li>Signposting and onward referring where necessary</li>  <li>Utilisation of the MECC principal</li>  <li>Attend and contribute to team meetings and events as required</li>  <li>Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role</li>  <li>Participate in regular health coaching supervision and continual learning</li> </ul>

<p>We are looking to recruit 4 Education Mental Health Practitioners in training to be part of a dynamic forward-thinking innovative team that works into schools and colleges.</p>
<p>The Mental Health Support Team (MHST) is part of a national rollout and has delivered successful result in the East Riding since September 2022. The MHST is part of the Thrive approach to Children and Young peoples mental health framework endorsed through the Emotional and Mental Wellbeing Academy.</p>
<p>The MHST supports schools , colleges and the wider system through advice , support , consultation, whole schools approaches staff raining and targeted interventions using CBT.</p>
<p>The successful candidates will embark on a one-year training course at an identified University to gain a post graduate diploma qualification. The role will be a combination of learning through an active caseload in service, university lectures, portfolio work and assignments<strong>. </strong></p>
<p>Successful candidates must be prepared to step into post January and February 2026.</p>



  <h3>Main duties of the job</h3>
  <p>The successful candidates will become part of a dedicated team of qualified Education Mental Health Practitioners and Senior Practitioners from multi-disciplinary backgrounds.</p>
<p>The successful candidates will access university for two days a week to undertake learning through face to face and online lectures and experiences.</p>
<p>Under supervision for two days a week the post holders will hold a small caseload to deliver outcome focused evidence-based interventions for children, young people and families experiencing mild to moderate emotional and mental health needs.</p>
<p>The training and service experience aims to equip the post holder with the necessary knowledge, attitude and capabilities to operate effectively in an inclusive, passionate innovative MHST.</p>
<p>The post holder must attend all university based taught and self-study days over a year that is required by the education provider, as specified within the agreed national curriculum and work in the service for the remaining 3 days of the week using their newly developed skills, knowledge and understanding supported by their supervisors and other qualified EMHP's.</p>
<p>On successful completion of the course post holders will move from a band 4 salary to a band 5.</p>

The post holder will collaborate with multidisciplinary teams, participate in research and audit activities, and adhere to Trust and departmental policies while staying updated on service and professional standards.</p> <p>There will be opportunities for delivering training to other staff as guided by their supervisor, ensuring compliance with record-keeping guidelines, and engaging in service-related research under supervision.
