The Recovery Hub does this, enabling early discharge from the hospital and reducing hospital admissions. The hub delivers complex rehabilitative care for the older adults in Leeds and the nursing care is delivered alongside a wider multi-disciplinary team. The focus always is on the individual, helping them progress to going home. 
We provide training, clinical supervision, and access to a peer support network. We won't just stop there though, we are proud of our career progression pathways that will allow you to develop your clinical, leadership, and preceptorship skills. Wharfedale Recovery Hub service works on a shift pattern and covers 24 hours, 7 days a week.

  <h3>Main duties of the job</h3>
  <p>ou will be a registered staff nurse and have excellent assessment, planning and evaluation skills to deliver clinical care to patients and support our Nursing Assistants.
You will coordinate and manage care ensuring that all calls are effectively assessed, triaged, allocated and recorded. You will have an overview of the service as a whole to ensure that patients and staff remain safe and care is appropriate and effective overnight. E.g. re-allocating work to cover unexpected staff absence.  At times you will be expected to work clinically and deliver direct patient care, though the majority of your role will be clinical triage via telephone. 
You must have excellent communication, organisational and IT skills as you will be coordinating care for a large number of complex patients.
You will provide clinical leadership, including line management of Band 5 Staff Nurses, and oversee line management of our Band 3 Nursing Assistants.
You must be able to think on your feet, problem solve and juggle conflicting priorities as well as motivate others in challenging times and utilise the resources available to you effectively.
The hours of work are 17:00pm-22:00pm over 7 days a week (including bank holidays).  The role is based from Hunslet Health Centre, though it is expected that you could travel across the city of Leeds during your shift to deliver patient care. 
It would be expected that you are available to work on a rota over 7 evenings per week, 365 days per year.

Then Leeds is the place for you!</p><p>This is an exciting opportunity to work within the MindMate Support Team (MHST) based in the City of Leeds. We currently have various EMHP posts available across the city due to lots of internal promotions as our service grows.</p><p>You will be employed as a Band 5 Education Mental Health Practitioner, working with colleagues across the city of Leeds, with education settings and health professionals to offer Mental Health and wellbeing support to young people up to the age of 18 and their families.</p><p>We will require successful applicants to be registered (or going through the process) with either the BPS or BABCP as you will be working within a NHS commissioned service.</p><p>You will work within LCH NHS Trust and provide a range of assessment and other interventions with individuals and wider systems under close clinical supervision from our experienced staff.</p><p>Regular case management and clinical supervision on an individual basis and peer group supervision will be offered to ensure excellent clinical standards and registration is maintained.</p><p>The MindMate Support Team holds a strong culture around wellbeing and work life balance and supports a hybrid model of working.
