We are looking for an experienced and passionate Care Navigator&nbsp;to work as part of our Prisons and Criminal Justice Directorate Team.Imagine being part of an organisation whose common purpose is to help those who are severely impacted by mental illness. We believe that everyone should be treated with respect and dignity – and that’s why equity is one of our core values.&nbsp;We draw on the expertise, unique perspectives and lived experience of our people – regardless of who they are or their background – to help us become inclusive and anti-racist employer, campaigning organisation and service provider that reflect the diverse communities we support as a mental health charity.Rethink Mental Illness work alongside Tees, Esk and Wear Valley NHS Foundation Trust (TEWV) and Humber Teaching NHS Foundation Trust to deliver an integrated stepped care mental health service within HMP Hull and Humber.The team compromises a range of highly skilled professionals who work collaboratively to provide the highest quality of care for those with mental health needs within the prison and for Rethink Mental Illness employees, this is carried out between 0800 – 1600 Monday to Friday.There may be some travel involved, but this will be done in work time and expenses paid to support service delivery.How you will make a differenceI provide a seamless package of support to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans. Reconnecting individuals with health services to reduce health inequalities and in turn reduce reoffending. The Care Navigators are embedded in integrated mental health teams and community settings, providing 1:1 and group assessment and support in a variety of settings and hold highly autonomous roles. Care Navigators, use a coaching framework, to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending. By preventing this decline in health, through robust reconnection with health services in the community we will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.Diversity, Equity, Inclusion&nbsp;Diversity is important to us and we appreciate difference through difference, inclusiveness and belonging.
