  <p></p><p>Sefton Enhanced Support Team is looking for an experienced Mental Health Practitioner to join our service.</p> <p>The Enhanced Support Team works with children and young people with complex needs who present with high risk, high harm behaviours and high vulnerability. They are likely to have difficulties in multiple domains which are persistent, severe and are framed by family and social contexts (family disruption, loss, ACEs). This may include for example, children and young people who are involved in youth justice services, not in education employment or training, who engage in substance misuse, have multiple adverse childhood experiences, who are neurodiverse or have speech language and communication difficulties etc. These children and young people are likely to have been referred to multiple services, but there are significant barriers to engagement</p> <p>Sefton Enhanced Support Team has links into Community CAMHS however is integrated with Children's Services including Social Care and Youth Justice.</p> <p>The successful applicant will provide a substantial contribution to the clinical work of the team and will hold case management responsibility for their workload. Provision of supervision to lower banded team members, students and other internal professionals as required is expected.</p> <p>We are particularly seeking candidates who have experience of working within complex systems, providing case consultation and working with families indirectly via wider professional involvement.</p>

If you are seeking a position where you can make a significant impact on the rehabilitation and quality of life to our client, this is the perfect role for you.</p><p><strong>The Role </strong></p><p>Supporting Hannah in her own home, encouraging and enabling her to fulfil her hobbies and interests whilst adhering to the care plan and implementing the therapy programmes. </p><p> We require Female Rehabilitation Support Worker(s) to actively encourage Hannah to engage in therapy programmes that will assist her in achieving goals to increase her independence and take on stronger maternal responsibilities, as a mother to her daughter.</p><p> <strong>Available Hours </strong></p><p>We are looking for <strong> </strong>Female Rehabilitation Support Worker(s) to undertake 24-hour shifts comprising day shift hours and sleeping night shift hours from 08:00 one day through to 08:15 the next day. Hannah needs some occasional help with hands-on personal care although help is generally required in a supportive capacity in this area. </p><p> Hannah lives at home with her husband and their dog who is very much one of the family . She is a bubbly lady with a great sense of humour, enjoying banter with those close to her and has a naturally gregarious and mischievous personality. Candidates will need to have a natural sense of fun and zest for life to keep up with Hannah s enthusiasm for living life to the full. </p><p> Hannah requires support in every area of her activities of daily living including; personal care, domestic organisation (washing/cleaning/cooking), mealtimes, mobilising safely, driving, scheduling her timetable and activities, using electronic devices and caring for her dog.</p><p> <strong>Hobbies &amp; Interests </strong></p><p>Hannah likes to be busy and enjoys being out and about in the community accessing leisure and recreational facilities, going shopping, for coffee with friends and spending time with her family, especially her young daughter. Hannah likes to present herself well; she has a particular interest in fashion and likes to have her nails manicured and painted regularly. She likes to make sure her home is cosy and comfortable.</p><p> Hannah also loves to go out in the evenings to bowling or bingo, going for day s out as well as enjoying short breaks and holidays within the UK and abroad.
