  <p>MHST EMHP / CWP/ Mental Health Practitioner</p>
<p>Hillingdon Mental Health Support Team is looking for a <strong>Qualified</strong> <strong>EMHP or Generic Mental Health Practitioner</strong> who has experience of working with Children and Young People in Mental Health Services in the provision of individual and group treatments. We seek to appoint an enthusiastic and vibrant individual who is passionate about improving health outcomes for children and young people.</p>
<p>Mental health professionals with post graduate training in a recognised therapy modality and experience of working with CYP may also apply. We are open to applications from newly qualified occupational therapists, mental health nurses, social workers or other relevant therapy post graduate qualifications.</p>
<p>You will work alongside other EMHPs, Mental Health Practitioners and a Senior Practitioner to support the wellbeing and mental health needs of schools and colleges in the borough of Hillingdon.</p>

  <h3>Main duties of the job</h3>
  <p>This is a role within the Green Paper for Transforming children and young people's mental health programme. The post holder will work within a service providing low intensity interventions. The post-holder will provide a range of cognitive behavioural therapy (CBT) based self- management interventions to children and young people with mild to moderate anxiety and depression and/or behavioural difficulties and their parent(s)/carer(s).</p> <p>The post holder will work with children, young people and parents with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities.</p> <ul>  <li>Undertaking assessments and delivering evidence-based, low-intensity, brief intervention for children and young people,with mild to moderate mental health problems, in an <strong>education setting.</strong></li>  <li>Helping children and young people within these settings who present with more severe problems to rapidly access more specialist service.</li>  <li>Supporting and facilitating staff in education settings to identify, and where appropriate, manage issues related to mental health and wellbeing.</li>  <li>Working with and within educational environments to afford better access to CYP mental health services.</li>  <li>Training and consultation to schools and other agencies about mental health in children and young people.</li>  <li>Support schools in developing their Whole School Approach to mental health.</li> </ul>

  <p>The CYP Eating Disorders Day Hospital Service is currently mobilising. This specialist provision delivers intensive, evidence-based care to children &amp; young people experiencing moderate to severe eating disorders. This innovative service aims to bridge the gap between inpatient and community care, offering a safe and structured alternative to Tier 4 admission &amp; facilitating earlier discharge from hospital settings.</p>
<p>Operating through a MDT working collaboratively to provide holistic, person-centred care, each professional brings their expertise to support physical health monitoring, nutritional rehabilitation, psychosocial interventions &amp; occupational engagement, all tailored to the unique needs of each young person &amp; their family.</p>

<li><strong>In-reach and Outreach support</strong> to prevent unnecessary hospital admissions &amp; reduce relapse.</li>
<li><strong>Intensive day hospital interventions</strong> focused on recovery, risk management, &amp; re-engagement with daily life.</li>
<li><strong>Meal support and psychoeducation</strong> for young people &amp; their families, promoting sustainable recovery at home.</li>

  <ul>  <li>To provide specialist dietetic service to children and young people with eating disorders (ED), alongside provision to outpatients as required</li>  <li>To undertake comprehensive assessments of each young persons diet and lifestyle to formulate individualised management and treatment plans ensuring optimal outcome whilst minimising risk.</li>  <li>To provide specialist advice to patients with eating disorders deemed to be a high physical risk</li>  <li>To be aware of and able to identify patients at high psychiatric risk (e.g. of suicide) and liaise with the MDT regarding the management of risk in these patients.</li>  <li>To act as a resource to junior members or non-registers support staff with placements/working within the EDDH</li>  <li>To write, implement and monitor guidelines for management of patients with eating disorders receiving outpatient care</li>  <li>To provide input for the training of student Dietitians.</li>  <li>To work autonomously to organise, provide and manage all CYP patients diagnosed with an eating disorder receiving multi-disciplinary team care while under the care of the CYP Eating Disorders Day Hospital</li> </ul>

We are looking for an experienced &amp; enthusiastic Education Mental Health Practitioner/Children's Wellbeing Practitioner.</p> <p>Youwill continue to build on the fantastic work carried out within your allocated schools.   <p>We are seeking an experienced and compassionate counsellor to join our team and help lead the delivery of a responsive and person-centred bereavement service.</p>
<p>The post holder will provide high-quality pre- and post-bereavement counselling to individuals and families, based on evidence-based practice and in line with statutory requirements and NHS policies. Working across hospital and community settings, you will support both adults and children, ensuring safe, individualised, and effective care.</p>
<p>This role also involves co-ordinating the day-to-day running of the counselling service, including the supervision and development of counsellors, students, and volunteers.   <p>The role involves day-to-day coordination of the Bereavement Support Service, including the management and support of a multidisciplinary team. The post holder will contribute to the ongoing development and delivery of high-quality bereavement care across the Hywel Dda University Health Board area, underpinned by research evidence, best practice, and clinical expertise.</p> <p>They will hold shared responsibility for managing a defined caseload independently, using recognised assessment tools and clinical judgement to deliver holistic, personalised therapeutic care.
