Opportunity for a full time Mental Health Practitioner to join our PCN team that emphasises patient care and staff training.

Your role will support 4 practice teams, ensuring a high level of care is delivered to all patients.

The PCN provides development support, with regular supervision and access to the PCN Clinical Education Lead.

We serve a patient list of approximately 48,000 across 4 GP practices, offering comprehensive primary care services.

The successful candidate will work across the GP practices in Congleton and Holmes Chapel, delivering triage, early assessment, brief intervention, improved access to appropriate care pathways and evidence based interventions for people experiencing early signs of mental ill health or long term conditions.

The role will also act as a bridge linking secondary mental health services, facilitating easier access to specialist care.

Further responsibilities include developing and implementing specialist mental health training across the PCN and, potentially, supervising junior colleagues and social prescribers.
